Physicals for 2021-22

**Remember a specific Sports physical is needed for many HS sports/activities.
 

These physicals are good for 3 years, but must be valid through the entire season (through the state meet) in order to be cleared.  Most kids get a sports physical before 7th and 10th grades. During the summer is the best time to get your physical so it does not interfere with any particular season. Smart School K-12 will NOT allow you to sign up if you are in need of a current physical.  Please turn in your sports physical clearance sheet to the school your child will be attending before 3:00pm. They will enter your physical and clear you to sign up for your sport/activity.
