Ronnie Gamble Scholarship Payout Request Form

Congratulations on all of your hard work!  The Ronnie Gamble Scholarship recognizes the athlete who the team could count on to give his best effort at practice, pitch in to help with team needs and encourage teammates to excel. 

Hard work, dedication, commitment, and spirit are the values Ronnie Gamble had and passed onto each Coach he worked with and each student he coached. We hope that you continue your efforts at your higher learning school of choice!

Scholarship Agreement:

This scholarship is a one time payment that is made available to Seniors only, annually through the funding from the Forest Lake High School Booster Club.  This scholarship is to fund continued education for the student/football player that is continuing their higher learning education.

As this is a one time award, this is scholarship is not available to the same recipient over multiple years and must be request within the 1st year of post-high school learning and only payable to a single higher education facility. A letter of admission will be required for proof of acceptance.

Payment of Scholarship Check: 

The scholarship check will be paid to both the recipient and the recipient's school of choice.   This award cannot be paid directly to the awardee for personal use.   

This payment will be mailed to your home address, it should be provided to the finance/bursar/admissions office to ensure proper deposit to your student account.
